Mission Statement
| To enhance the quality of life for people with developmental disabilities in Weld County, Colorado. |
Program Summary
| Envision services assist children, adults, and their families who have a developmental delay/disability, i.e. mental retardation, cerebral palsy, epilepsy, spina bifida, autism or other neurological disorders.
Envision offers case management, adult day services (including activity services and employment services), adult residential services, supported living services, children’s extensive support services, early intervention services, transportation services, family support and administration services. Services are based on individual needs. Envision programs are not entitlements, but provided on a funds-available basis only. |

| Envision is currently looking for an individual or organization to cover the cost of a replacement battery for an electric hoyer lift. The cost for the battery is $320. Thank you for your interest and support! |

| Tony's Story:
Long-term case management allows Envision to help individuals through good times and bad. When Tony first came to Envision, he was a vibrant, active man who required few formal services. His circumstances changed dramatically after he became extremely ill, with little hope of ever leaving a nursing home. Envision was not willing to give up on him. Using Tony’s previous lifestyle as a guide, Envision implemented services that have restored a great deal of the quality and vitality of Tony’s pre-illness life. He is once again living in the community, and is reacquainting himself with the people and places that mean so much to him. This first-hand knowledge of Tony's past was instrumental in helping him shape a fulfilling future. |
